2025 FALL BASELINE CONCUSSION TESTING - Summer Testing for all (if you did not test in May)
Baseline Concussion Testing for Century Fall Sports will be at Century HS in the North Cafeteria on Thursday, July 31, 2025. Mandatory for all soccer players.
9:30 am - Girls Soccer
This IS MANDATORY& needs to be done EVERY school year)
There will be two parts to this test, one on their phone and the other on the computer with one of our testers.
Download the Sway Medical app on your phone prior to the testing date and have their phone fully charged for test day.
You do not need to do anything with the SWAY app besides downloading it.

Baseline concussion testing needs to be completed BEFORE the student can practice/tryout on August 11. NO EXCEPTIONS. There will not be testing on August 11.

A copy of your physical needs to be on file in the Athletic/Activities Office PRIOR to the registration date listed or the system will not let you register. You can locate your physical date in Skyward Family Access
(under the tab on the left - report cards/portfolio)
IF your physical expires between August 11 and November 24th, an updated copy needs to be submitted to the Athletic/Activities Office.
DO NOT WAIT to schedule your physical. A copy of your physical NEEDS to be in our office NO LATER than Friday, Aug 1 in order to register online.
1. A copy of your physical is on file in the Athletic/Activities Office – PRIOR to registering online.
2. Make sure your registration is completed online
3. Fee has been paid
4. Have completed concussion testing.
You will not be able to participate or tryout if the above steps are not complete.

Create an account-
1) Orders are placed for weekday away games only. No Saturday games or home games.
2) Orders must be placed 24 hours in advance - deadline: 5:00 the night before.
3) Where it says first name - include your whole name. Where it says last name - please write "Century Girls Soccer" AND include the team (Varsity, JV, B, 9) so we can make sure the sandwiches are placed on the correct bus.
4) If you have more than 1 child playing, please order the sandwiches separately so that they are put on the correct bus.
5) DO NOT ORDER BEVERAGES! BRING YOUR OWN.
6) Order mayo or mustard on the side to keep it fresh.

August 11 2025- tryouts start for everyone. Come prepared
We compete at 3 levels: 9th grade, ,JV and Varsity. We do not have a middle school team. 8th graders can try out.
THERE WILL BE GAMES/ PLAYOFFS for Varsity OVER MEA - DO NOT SCHEDULE EVENTS

Everyone should report to the SE field located behind the stadium scoreboard; between the baseball and softball fields.
